Post-Rehabilitation Support
Returning to everyday life after rehabilitation can be challenging. We provide practical, non-judgemental disability support to eligible NDIS participants rebuilding their routines, confidence and independence following physical rehabilitation, injury, illness or drug and alcohol rehabilitation programs.

Appointment & Community Transport
Getting to where you need to go is an important part of living independently. We assist participants to access medical appointments, allied health services, community programs, recreational activities and other destinations — providing safe, reliable transport support as part of a broader disability support arrangement.

NDIS Funded Support
All HealthZen services are available to eligible NDIS participants. We work with participants, families, support coordinators and plan managers to ensure your support is delivered in line with your NDIS plan goals and funding categories.
